The HDSP will continue to work on awareness of high CVD risk in diabetics, and support DPC in achieving their goals and objectives.
The lifestyle issues and established interventions discussed in the Prevention of Risk Factors section (particularly Winners Circle, Eat Smart Move More, Healthy Weight in Children and Youth, BASIC, Starting the Conversation Tools, Quit Now NC!) are critical in the prevention and management of diabetes. Refer to these sections for objectives and strategies to reduce these risk factors.
Refer to the prevention and management issues in the other Management of Risk Factors sections (Hypertension, High Cholesterol, and Obesity) for prevention and management of diabetes.

- Use the Chronic Care Model to deliver quality diabetes outpatient care, through the existence and expansion of the State-Based Collaboratives.
- Encourage health care providers to provide lifestyle behavior change counseling or instruction for those with pre-diabetes and those with diabetes i.e. Starting the Conversation Tools (Lead Agency: NCPP).
Professional Education
- Raise awareness of pre-diabetic condition among health care providers to encourage early interventions.

- Use the Chronic Care Model to expand State Based Collaboratives to address management of hypertension and cholesterol for the prevention of heart disease and stroke.
- Foster shared objectives and initiatives i.e. collaboration on grants and projects, such as HOT2.
- Develop key partnerships to establish program services reaching minorities i.e. CDC’s Diabetes Today Model. (Key Partnerships: General Baptist State Convention with African American churches; the NC Commission of Indian Affairs; El Centro Hispano; the NC Office of Minority Health targeting African American and American Indian faith-based groups) This belongs under Responsible parties and partnering organizations
- Create programs which promote community collaboration and sustainability, while improving quality of life of underserved individuals with, or at risk of developing, diabetes. Lead Project: Project IDEAL (Improving Diabetes Education Access to Care and Living). Supported by DAC and administered at Wake Forest University, School of Medicine.
Public Awareness
- Use established community-based partnership models to raise awareness of diabetes and its complications among an at-risk population i.e. Project DIRECT (Diabetes Intervention Reaching and Educating Communities Together).
- Link with national and other chronic disease campaigns i.e. The National Diabetes Education Program’s "Be Smart About Your Heart Control the ABC’s of Diabetes (A1c, Blood Pressure and Cholesterol)".(Lead Agencies: DPCP, MRNC and HDSP)
